As players close Tears and defeat enemies inside a Rupture, rewards continue to scale. Special Rupture Goblins can also appear, creating additional Ruptures and extending the action even further.
Realmwalkers Are Back
Originally introduced during Season 6, Realmwalkers return in Season 14 as a key part of the Pandemonium Rupture system.
These massive enemies function as world-boss-style encounters that require significant damage output and survivability to defeat. Summoning one involves clearing guardians around Death's Head Idols and completing the associated Rupture event.
Not every Rupture leads to a Realmwalker:
Normal Ruptures never summon one.
Surging Ruptures have a chance to spawn one.

Defeating a Realmwalker opens access to the Deathtoll Chamber, a compact endgame activity packed with valuable loot.
Players can also access the chamber through Nightmare Dungeons featuring the Rupture affix after meeting certain seasonal requirements.
Although relatively small in size, the Deathtoll Chamber plays an important role in Season 14's progression system. It serves as one of the primary sources of Greater Lair Boss Keys, which are required to unlock high-level seasonal rewards in Torment difficulties.
